This survey is completed every semester by all students working in the Tax Clinic.
A Formidable form, "stock" except for custom HTML layout.
The biggest issue is that there are columns included for the radio button labels, which as all blank.
Does work on mobile (tested on Android/Chrome), not responsive, a bit awkward - WYSIWYG editor is a bit too wide.
Once the user info column is removed (as well as the empty label columns), the results need to go to Ron Blasi - Tax Clinic Director.
To Do
Does not currently limit only to Tax Clinic students - all students, faculty, staff can access, so will need to confirm all submissions are from clinic students (Law 7600, 7601)
Perhaps could develop a view to dump all the data without the user info.
